1. Waiver of Liability: I, the undersigned, do hereby waive, release, and
discharge all claims for damages, death, personal injury which may occur or
which may hereafter accrue to my child as a result of participation in Waiver of
Liability: I, the undersigned, do hereby waive, release, and discharge all claims
for damages, death, personal injury which may occur or which may hereafter
accrue to my child as a result of participation in Vazball Basketball Camps.
Knowing the risks of the activity, I hereby agree to assume those risks. This
release is intended to discharge and hold harmless Vazball Basketball Camps,
and it’s employees from liability. This waiver and assumption of risk is to be
binding on my heirs and assigns. I further understand that photographs may be
taken of my child during the course of the camp and these may be used in
Vazball Inc. publications. Please consult a physician before beginning any type
of exercise program.
2. Refund Policy: VAZBALL Basketball camp will withhold 20% of the total camp
fee for administrative costs on all refunds.
3. By signing and/or agreeing to this assumption of risk, liability release, and
refund policy statement, I acknowledge that I have read it’s content and
disclosures, and I agree to it’s terms.
4. VAZBALL Basketball camp provides equal opportunity regardless of race,
creed, or sex.
